Job Description
Description
As a Senior Forward-Deployed Engineer at Bask, you will work directly with the CEO and CTO to build and design the software infrastructure that will serve as the backbone of Telehealth. Helping lead frontend migrations, performance work, and production AI builds.
We are looking to expand our software engineers, seeking those with deep rooted entrepreneurial spirit. Success looks like production code live in customer environments, customer teams able to extend it on their own, and a wider partnership with each account.
Our serverless platform is a multi-tenant monorepo — Next.js and React on the front, tRPC and Prisma behind it, deployed on SST, TypeScript throughout. One engineer can follow a change from a form field to the database without crossing a team boundary. That's the point.
Engineering at Bask Health is AI-first and builder-led. Work starts with general understanding of problem, moves to tools like Claude and Cursor for further investigation, making a plan and building, tested on staging with realistic data, released to production, tested with real patient and care teams, and then back for systems and polish. AI is a default part of how we work, not a side experiment — because moving fast in healthcare requires every advantage.
We are looking for engineers who take end-to-end ownership, treat AI as a real collaborator, and care deeply about building software that meaningfully improves how healthcare is delivered.
What You'll Do
- Design, build, and ship high-quality product experiences. Own key surfaces end to end, from concept through production. Write elegant, well-tested code — we have a real test suite and we intend to keep it that way.
- Work across the stack. Follow a feature from the interface through the API to the database. No handoffs, no waiting on another team.
- Work AI-first with Cursor and Claude Code. Use LLMs as your starting point. Draft intent, explore implementations, scaffold components, and refactor with AI in the loop. Apply your own judgment to meet the quality bar healthcare demands.
- Prototype fast and validate with real users. Build and test quickly with real patients and care teams. Use what you learn to refine before fully committing — especially in sensitive clinical contexts where getting it right matters.
- Build for scale and own the codebase. Translate product needs into reusable, well-architected patterns that make the next feature faster to ship.
- Make automation legible and trustworthy. Build systems that clearly communicate what AI and automation are doing on behalf of patients and providers. Earn trust through transparency, reliability, and thoughtful defaults.
- Champion performance, accessibility, and reliability. Ship work that is fast, robust, and accessible. Partner across the team to debug, optimize, and raise the bar over time.
- Share AI-native workflows. Document prompts, patterns, and workflows that work, so we move faster together without cutting corners that matter to patient outcomes.
We are a highly collaborative team.
Requirements
A full-stack generalist who has taken a product from 0 to 1 and can apply that instinct inside a system that already has real patients on it. Entrepreneurial, willing to roll up their sleeves, and drawn to a mission that disrupts how healthcare gets delivered.
- 3+ years of experience
- You are dangerous on the frontend of the stack.
- You are a highly skilled dev (React, TypeScript, Next.js, Tailwind and JavaScript).
- You also have backend skills (tRPC, Drizzle ORM, SQL, Node, SST).
- You can work independently and are comfortable owning entire projects or large areas of the codebase.
- Strong experience building paid products and services is preferred.
- React Native or Expo experience is a plus.
- You are a strong communicator, able to clearly articulate technical problems, constraints, and timelines.
- You are comfortable working in a fast-paced, unstructured, startup environment.
๐ฏ Who is this job for?
This role is suitable for a Senior full-stack engineer with 3+ years of experience, an entrepreneurial mindset, and the ability to independently own projects from concept through production. Required skills include strong React, TypeScript, Next.js, Tailwind, and JavaScript expertise, plus backend experience with tRPC, Drizzle ORM, SQL, Node.js, and SST; React Native or Expo is a plus. The candidate should be familiar with building and scaling paid products, leading frontend migrations, developing production AI features, working across the full stack, optimizing performance and accessibility, and delivering tested software in fast-paced startup environments.
๐ฌ Potential Interview Questions
How would you design an end-to-end feature in a multi-tenant Next.js, tRPC, Prisma, and TypeScript monorepo?
I would trace the feature from the React UI through a typed tRPC procedure to the database schema and migrations, enforcing tenant isolation at every layer. I would also add validation, authorization, tests, observability, and a staging workflow using realistic data.
How would you prevent data leakage between healthcare tenants in a multi-tenant application?
I would derive the tenant context from the authenticated session rather than trusting client input, then enforce tenant scoping in server-side procedures and database queries. I would add authorization tests covering cross-tenant access attempts and review indexes, caching, logs, and background jobs for isolation issues.
How would you optimize performance in a Next.js and React application used by patients and care teams?
I would measure first using browser profiling, Web Vitals, server timings, and database query metrics. Then I would address the largest bottlenecks through appropriate server rendering, caching, code splitting, minimized client-side JavaScript, optimized queries, and efficient loading states.
How do you structure and validate a tRPC API backed by an ORM and SQL database?
I would define strongly typed procedures with explicit input validation, authentication, authorization, and predictable error handling. Database access would use transactions where needed, carefully designed indexes, parameterized queries through the ORM, and integration tests against representative data.
What is your approach to database schema changes and migrations in a production healthcare system?
I would make migrations backward compatible whenever possible, deploy additive changes before switching application behavior, and avoid long-running locks on critical tables. I would test migrations against production-like data, define rollback or recovery procedures, and monitor the release closely.
How would you deploy and operate a serverless TypeScript application using SST?
I would define infrastructure as code with clear separation between environments, least-privilege permissions, secure secrets management, and reproducible deployments. I would also configure logging, metrics, alarms, tracing, and staged rollouts so failures can be detected and recovered quickly.
How would you test an AI-powered workflow that affects patients or healthcare providers?
I would combine unit and integration tests with curated evaluation datasets, adversarial cases, human review, and monitoring of production outcomes. The system should expose uncertainty and automation boundaries, preserve auditability, and require appropriate human confirmation for high-risk actions.
How would you make AI automation transparent and trustworthy in a clinical product?
I would clearly identify generated content, explain what data and actions the automation used, and provide users with review, correction, and override controls. I would log relevant decisions and outputs while protecting sensitive information and continuously evaluate accuracy, safety, and bias.
How would you improve accessibility and usability in a React and Tailwind application?
I would use semantic HTML, keyboard-friendly interactions, visible focus states, correct labels, accessible error messages, and appropriate ARIA only when necessary. I would validate with automated tools and manual testing using screen readers, keyboard navigation, responsive layouts, and realistic user workflows.
How do you use tools such as Cursor and Claude Code without compromising code quality or security?
I use AI to explore unfamiliar code, draft implementations, generate tests, and suggest refactors, but I retain ownership of design and review every change. I avoid exposing sensitive data or secrets, verify generated code against requirements, run the full test suite, and inspect authorization, performance, and failure behavior before release.
